Subcutaneous Emphysema by Clonoscopy

Abstract

|
A 65-year-old woman was admitted for severe subcutaneous emphysema after colonoscopic exam. It was accompanied by dyspagia, voice change and mild abdominal distension. The chest X-ray showed pneumomediastinum, pneumopericardium and broad subcutaneous emphysema in thorax. After she was admitted, mild fever and abdominal pain occurred. Emergent abdominal CT revealed pneumoperitoneum, pneumoretroperitoneum, fluid collection, which made to consider a large bowl perforation by colonoscopy. She was taken to an emergent operation. The subcutaneous emphysema of thorax disappeared naturally after 3 days. She was discharged uneventfully and followed up for 6 months. As in this patient, cases of the large bowl perforation by colonoscopy, which were accompanied by chest subcutaneous emphysema, pneumomediastinum, pneumopericardium, pneumoperitoneum and pneumoretroperitoneum have rarely been reported. Therefore we present this case with literature review.
